A civil partner who bears a civil partnership name may, after dissolution of the civil partnership (dissolution or death of the other civil partner), resume his/her maiden name or the family name used until the civil partnership name was determined.

- Current certified copy of the civil partnership register, if the register is not kept at the registry office where the declaration is made
- Final divorce decree or death certificate

The declaration can be submitted either to the registry office of the place of residence or to the registry office where the civil partnership was established.
If the declaration is made at the registry office of the place of residence, the latter will forward a certified copy of the declaration to the registry office that registered the civil partnership at the time.
The declaration of re-acceptance is processed at the registry office where the civil partnership was established.
