If a foster care relationship ends, follow-up care or transitional care can be provided for young adults. This can also be provided by the previous carers. Agreements on this are made in the care planning.
When a foster child leaves a foster care arrangement, this is referred to as a “leaving care process”. This is supported by the local foster care service.
The goals of aftercare include, for example, the foster child becoming independent once they reach adulthood or the successful transfer to another form of youth welfare. It can also be possible to facilitate a return to the family of origin or to support an adoption.
The specialists of the local foster care service accompany the foster children and the carers, among other things through reflective follow-up discussions.
Counselling should be provided within a reasonable period of time after the end of the assistance in a form that is understandable, comprehensible and perceptible for the young adults.
Aftercare is agreed upon as part of the assistance planning between the youth welfare office, the young adult and, if applicable, his or her legal guardian.
