Has someone in your company or building been seriously injured or even died while using certain work equipment? Then you have to show this.

If an accident occurs during the use of work equipment and someone is seriously injured or killed, you as the employer must report this to the responsible authority.

These specific work equipment includes systems that require monitoring such as,

• Cranes
• LPG systems
• Mechanical equipment for event technology
• Elevator systems
• Printing systems
• Systems in potentially explosive areas

Informal notification with the following information:

• Company address
• Address of the business premises or the construction or assembly site at the event location
• Date of event
• Type of event e.g. explosion, media leak, crash
• Number of injured persons, number of killed persons
• Equipment involved
• included approved monitoring body (for systems requiring monitoring according to Annex 2 of the Ordinance on Industrial Safety)
• involved body for determining the cause (for work equipment according to Annex 3 of the Industrial Safety Ordinance)
• Brief description of the event and the damage that occurred (including photos and videos) measures already taken by the employer

The competent authority may also request the following information from you:

• Risk assessment
• Proof that a qualified person has prepared the risk assessment
• Information about the responsible persons
• Information on protective measures taken
• Evidence of regular training of employees
• Operating instructions

• Mechanical equipment for event technology

There are no costs involved.

• You shall immediately submit the report of the event in writing or by email to the competent authority with all necessary information.
• The ad is being reviewed.
• The competent authority will inform you whether further documentation (e.g. an assessment by an approved inspection body) is required.
• The accident will be investigated if necessary.

The competent authority will report immediately.

You must report the accident immediately.

There are no hints or special features.

No legal remedy provided.

• Notification of the use of work equipment in accordance with the Industrial Safety Ordinance Receipt in the event of an accident
• Industrial accidents in which a person is injured or killed must be reported to the occupational safety authority
