Adapted to the personal situation of those seeking advice and help, a needs-based solution for care and relief care - if necessary together with the relatives or caregivers - is to be found and mediated by means of advice and possible assistance.

People in need of care or at risk of needing care, as well as their relatives, are given free, neutral and professional advice at the local care support centers on all issues relating to home care, part-time inpatient care (e.g. day or night care), short-term care (short-term home care, e.g. after a hospital stay) or inpatient care (permanent home care). On request, help is provided with the application for care benefits, or appropriate assistance is arranged or contacts are established. Competent care advisors and social advisors are available in the care support centers for this purpose. The care support centers are operated jointly by the care insurance funds and the respective administrative district or the respective independent city. As a rule, they are located in the vicinity of the district or municipal administrations of the respective counties and independent cities.
The state supports the care support centers together with the state associations of long-term care insurance funds and the municipal state associations within the framework of a steering committee.

You do not need any documents for the general consultation. Possibly only in consultation with the care support point.

Presence of need for care or threatened by need for care

There are no fees.

The nursing centres can be visited directly by the counsellors during public office hours or can be consulted by telephone. You can also contact us by e-mail or fax.
On request, the consultation can also be provided at home with the patient. For this purpose, a telephone appointment with the nursing home is necessary beforehand.

The nursing centres will also advise you in your home if desired.
The nursing centres are not responsible for:

• Questions of the cash supervision (contact person of the competent ministry of the headquarters of the respective care fund),
• for questions of home supervision (responsible home supervision of the district or the district-free city),
• Questions about the care housing benefit (stock regulation - social office of the district or the district-free city),
• for questions of the recognition of low-threshold offers (State Office of Health and Social Affairs - LAGuS M-V).
