Initial application for determination of the severely disabled status according to Neuntes Buch Sozialgesetzbuch (SGB IX)

If, due to an illness, an accident or from birth, you have a significant condition that restricts your participation in life in the community, you can have this restriction officially determined as a disability. The disability must not be temporary, but must last longer than six months.

In addition to the determination that a disability exists, its extent (degree of disability/ Grad der Behinderung/GdB) is also determined and the marks for claiming disadvantage compensation are assigned.

Use the application form. You can also obtain a paper copy of the application form from the competent office.

You can speed up the assessment procedure by enclosing comprehensive medical reports with a precise description of the findings or the examination documents held by your family doctor (e.g. specialist letters, hospital reports, course final reports, X-ray diagnostic findings) with your application.

Medical certificates that only contain the complaints expressed are not sufficient. Reimbursement of costs for submitted medical certificates cannot be guaranteed.

It is possible that your medical documents are not sufficient for a decision. In this case, you must expect a medical examination and assessment. If necessary, you will be invited to an examination in writing.

Costs may be incurred for the medical certificates and attestations submitted, which you will have to pay by yourself.

You make the application in writing on the prescribed form or you give it orally to the competent office for recording.

• Call up the application form "Amt24" or request a paper copy from the competent office.
• Complete the application in full, making sure above all that you enter your attending doctors and sign the application.
• The competent authority will be available to answer your questions and help you complete the application.
• After examining your application, the competent office will issue a notice of assessment, which will state the individual disabilities, the degree of disability (GdB) and the other health characteristics (characteristic signs).
• This decision will also be issued if the GdB is less than 50.
• If the responsible office has determined that you have a GdB of 50 or more, you will receive a severely disabled person's card. The relevant markings and the period of validity are entered on the back.

Note: People are considered severely disabled if they have a GdB of at least 50 and live in the Federal Republic of Germany or have their usual place of residence here or are employed here.

A severely disabled person often has several disabilities at the same time (multiple disabilities). The disabilities can exist independently of each other, but they can also overlap and reinforce each other. The mutual relationships of the individual disabilities are taken into account when determining the degree of disability (total GdB).
