If your driving licence has been lost or destroyed, you must report the loss to the relevant driving licence authority immediately and apply for a replacement document.

If you find the lost or stolen driving licence after the replacement licence has been issued, you must hand it in to the driving licence authority.

• if applicable: Proof of the theft report to the police
• identity card or passport with current registration confirmation
  (alternatively a current confirmation of personal data by the registration office)
• biometric photo
• if applicable: Extract from the local driving licence register of the driving licence authority that issued your driving licence
  (The so-called "index card copy" is only required if your previous driving licence was not issued in the jurisdiction of the now competent driving licence authority and was issued before 01.01.1999)

• in the event of theft: report the theft to the police
• in the event of loss: report the loss to the driving licence authority (as a rule, an affidavit is required for a fee)

• Issue of the replacement driving licence: EUR 19.20 to EUR 37.10 (depending on administrative costs)
• if applicable: Taking of an affidavit in lieu of an oath by transcript at the administrative authority: EUR 30.70
• if applicable: replacement certificate until the new licence is issued: EUR 7.70

You must submit the application in person to the driving licence authority.
