Since 1 January 2013, the severely disabled person's ID card has been issued in the form of an identification card (cheque card format). An extension is therefore no longer possible for technical reasons. If your severely disabled person's ID card expires, you can apply for a new severely disabled person's ID card.

If you still have an old severely disabled person's identity card in DIN A6 format made of paper, this will remain valid until it expires. However, this will no longer be extended, but will be replaced by a new severely disabled person's ID card in cheque card format when it expires.

• Documents proving the disability (assessment notice in accordance with the law on severely disabled persons).
• passport photo, if applicable
  • Exception: In the case of severely disabled persons who cannot leave the house or can only do so with the help of an ambulance, the ID card will be issued without a photograph on application. The issuing authority will then note "valid without photo" on the ID card.
• for non-EU citizens: valid residence permit

The competent authority may request further documents from you if necessary.
