If you have been assessed as having a degree of disability of at least 50, you can apply for a severely disabled person's pass.

Depending on the chances of improvement in your state of health, this will be issued for a limited period of up to five years or indefinitely.
If you are a severely disabled person with a temporary residence permit, the ID card can only be issued for as long as the residence permit is valid.
The severely disabled person's pass enables you to prove that you are severely disabled and indicates any special health restrictions. You can use it to claim special rights and corresponding compensation for disadvantages.
In the event of expiry, loss or change of other important data, you must apply for a new severely disabled person's ID card.
- Assessment notice of disability of at least degree 50 or corresponding reference number
- Photo the size of a passport photo (from the age of 10)
- Valid residence permit in Germany (for non-German citizens)
You can only apply for a severely disabled person's pass if you have previously been assessed as having a degree of disability of at least 50.
If you apply to the competent authority for a disability certificate, you will automatically be issued with a severely disabled person's pass if you have a degree of disability of at least 50, provided you already have a passport photo.
If this is not the case,
- you already have an assessment notice or a corresponding administrative or court decision with a degree of disability of at least 50
- or if your existing severely disabled person's ID card is lost, damaged or expires,
you can apply to the relevant office for a (new) severely disabled person's ID card. An informal letter is sufficient for this.
You must send a photo/passport photo of yourself to the relevant office for the new card to be issued. Please note your name and the authority's reference number on the back of the photo to avoid any confusion. The competent authority will check the requirements for issuing the severely disabled person's ID card.
If the requirements are met, it will issue a (new) severely disabled person's ID card and send it to you.

The new issue of the severely disabled person's ID card is carried out by the locally responsible Hessian Offices for Care and Social Affairs
